چکیده

With the greatly increased interest in neonatal morbidity in recent years it is now realized that pneumothorax and pulmonary interstitial emphysema in the newborn period, for many years regarded as medical curiosities, are not uncommon. Indeed, Davis and Stevens (1930) demonstrated the presence of pneumothorax radiologically in 1% of 702 consecutive babies and Solis-Cohen and Bruck (1934) in approximately 2% of 500 neonates. While the majority recover spontaneously, in others the condition progresses rapidly and death may ensue. The following cases illustrate the association between obstructive atelectasis, pulmonary interstitial emphysema and pneumothorax. In addition, two fatal cases of a congenital anomaly of the lungs (pulmonary alveolar dysplasia) are described in both of which pulmonary interstitial emphysema developed. Pulmonary alveolar dysplasia does not appear to have been recorded before as a cause of interstitial emphysema and pneumothorax in the newborn.

Spinnaker Sail Sign Accompanied with Pneumopericardium and Pneumoperitoneum

Air-leak syndromes include pneumomediastinum, pneumothorax, pneumopericardium, pneumoperitoneum, pulmonary interstitial emphysema and subcutaneous emphysema. Pneumothorax and pneumomediastinum occur in 1 to 2% of normal neonates and they usually has no symptoms and resolves spontaneously. Here we report sudden fatal pneumomediastinum accompanied with pneumopericardium and pneumoperitoneum in no...

Evaluation and treatment of emphysema in a preterm infant

Infants delivered prematurely often have respiratory distress related to surfactant deficiency or pneumonia. The current case describes a premature neonate who presented with increased work of breathing and recurrent left pneumothorax. Although chest x-ray showed left upper lobe changes consistent with pulmonary interstitial emphysema, subsequent CT scan performed due to recurrent pneumothorace...

Pulmonary interstitial emphysema in an adult with metastatic choriocarcinoma

Although typically seen in neonates, barotrauma is a well-established complication of mechanical ventilation in other populations as well, particularly associated with the use of high peak pressures and continuous positive airway pressure. Complications range from subpleural bleb development to life-threatening pneumothorax. We report a case of barotrauma in an adult that initially presented as...

Exogenous surfactant therapy in newborn infants.

Exogenous surfactant therapy has an established role in the management of neonatal respiratory distress syndrome (RDS). This article summarises the current evidence on surfactant therapy. The use of surfactant for the treatment or prophylaxis of neonatal RDS results in a 30% to 65% relative reduction in the risk of pneumothorax and up to a 40% relative reduction in the risk of mortality. Advers...

Development of Localized Pulmonary Interstitial Emphysema in a Late Preterm Infant without Mechanical Ventilation

Pulmonary interstitial emphysema (PIE) is not an uncommon finding in premature infants with respiratory distress who need respiratory support by mechanical ventilation. PIE has been reported in a few cases of neonates in whom either no treatment other than room air was given or they were given continuous positive end-expiratory pressure (CPAP) support. We present a case of a premature neonate w...
